Join us in making a difference in this part-time, temporary position, where you will receive a paid meal period, lunch, snacks and a gas stipend.</strong></p><p><strong>POSITION SUMMARY:</strong> The Surveyor/Implementor reports to the Executive Director and Managing Director, Healthy Communities – Permian Basin. Texas Tech University Health Sciences Center in collaboration with Beacon Alliance and the Permian Strategic Partnership is implementing a population health initiative to improve the health and well-being of rural communities in the Permian Basin (West Texas and Southeast New Mexico). The Healthy Communities – Permian Basin program aims to define the depth and breadth of undiagnosed and undertreated health conditions in rural areas and bring innovative, person-centered solutions to community members to improve health outcomes. The initial program is based in Crane, Texas.<br></p><p>The Surveyor/Implementor, as part of the Healthy Communities – Permian Basin team, assists in bridging the gap between healthcare systems and rural communities across the Permian Basin region of West Texas and Southeast New Mexico. The Surveyor/Implementor will assist with collecting data and biomarkers to help identify community health needs. The Surveyor/Implementor will work closely with other members of the Healthy Communities – Permian Basin team.</p><p>The ideal candidate(s) will spend the majority of their time in the field interacting with community members. They will travel to a variety of settings, including homes, churches, or community facilities to meet with community members. The work schedule will include evenings and weekends to attend community events, programs or meetings, or to interact with community members according to their availability.</p><p>We are actively hiring qualified candidates for this position. This role is considered a temporary part-time, non-exempt position. This position is temporary in nature and is expected to end at the conclusion of the assignment. Candidates may have opportunities for continued employment beyond the assignment end date but it is not guaranteed.&nbsp;</p><p>Key responsibilities of the Surveyor/Implementor include:</p><ul><li>Conduct outreach activities to promote health awareness and preventative measures.  </li><li>Conduct door-to-door household program.  </li><li>Provide culturally appropriate health and informational resources. Understand and respect the unique cultural needs and values of the rural communities they serve.  </li><li>Conduct basic health screening activities such as measuring community members’ blood pressure and weight; finger prick and HbA1c testing; and install Continuous Glucose Monitor.  </li><li>Increase individual and community participation in assessments and education to improve health and well-being.  </li><li>Identify health-related issues, collect data, and report concerns to Healthy Communities – Permian Basin program directors.   </li><li>Complete administrative and technical training prior to field work.  </li><li>Demonstrate competency in conducting survey related activities.  </li><li>Identify all eligible respondents in the selected households and administer relevant questionnaires.   </li><li>Manage technology such as Wi-Fi hotspots and tablets.  </li><li>Set up and manage health-related mobile apps for community members.  </li><li>Maintain information on completion of surveys and biomarkers for follow-up.  </li><li>Work in non-traditional settings e.g., outside, homes or vans.  </li><li>Participate in debriefs.  </li></ul><p><strong>Qualifications:</strong></p><p>Education:  </p><ol><li>High school diploma or equivalent required; and  </li><li>Enrollment in an advanced degree program in a health-related field, e.g., nursing, physician’s assistant, psychology, public health or social work strongly preferred.</li></ol><p>Communication:</p><ol><li>Effective communication and relation-building skills.   </li><li>Bi-lingual in Spanish and English preferred.  </li><li>Emotional maturity; demonstrated cultural sensitivity to interact with diverse populations.  </li><li>Demonstrated ability to deliver health-related information.</li><li>Demonstrated ability to work within a team setting.</li></ol><p>&nbsp;Other:</p><ol><li>Must be 18 years or older</li><li>Basic knowledge of diabetes and hypertension.</li><li>Excellent organizational skills.  </li><li>Desire to improve health outcomes for communities in rural United States.  </li><li>Must be available to work Thursday-Sunday schedule.</li><li>Current driver’s license and automobile insurance.</li></ol><p><strong>Supervision Requirements:</strong> There are no supervision requirements with this position.  </p><p><strong>Physical Requirements:</strong> Climbing, lifting, balancing, walking, stooping, and handling materials may be a part of the activities of their work. The Surveyors/Implementors often need to stand, walk, and use their hands to operate a computer and may lift and move up to twenty-five (25) pounds. The Surveyor/Implementor may need to climb stairs or ramps to access certain areas. The Surveyor/Implementor needs to drive to various locations for their work.</p>]]></media:description></item></channel></rss>
